Before Your First Visit
A few quick steps make check-in faster:
- Fill out your patient forms online before you arrive.
- Bring a photo ID and your dental insurance card, if you have one.
- Have a list of your current medications ready.
- Bring recent X-rays from your previous dentist if you have them. If not, we can request them for you. Just let us know who you saw last.

Keen Care Plan
No dental insurance? For $40 a month, or $400 a year, our membership plan includes two cleanings and two exams a year, X-rays as needed, and 25% off any treatment you need. Families save up to 15% more.
